Food Allergy Reaction Report

Welcome to our Food Allergy Reporting page. This page is intended to encourage Food Hypersensitive [FHS] customers and Food Business Operations [FBO’s] to report food allergy incidents resulting from eating out in all sectors of hospitality, Hospitality, hotels, restaurants, cafes, event catering and festivals, education settings (nurseries, schools, colleges, activity groups and universities).  Hospital catering – travel airlines, airports, boats,  trains and their terminals.

Note:  You can also include within education settings, issues within their curriculum or classroom environment.

Why are we collating the information?

We know that currently there is no single point or method to report food allergy reactions, and no central database to monitor the number or severity of incidents within the UK.  Each Local Authority operates independently and does not currently collate data centrally, unless there is a severe reaction or fatality. Our aim is to create a centralised database of reports from customers and FBOs.

We are also aware that FHS consumers struggles to understand how to report an incident or where to report the incident to. This results in a lack of information about the types of incidents and why they continue to happen on a regular basis.

Please note we will NOT be investigating individual cases, however; if the team believes that an FBO has seriously breached food safety laws, or we see a pattern emerging, we would have a duty of care to contact the FBO and/ or Local Authority as appropriate about potential risks form the information supplied. Where the applicant has supplied contact information we will contact to discuss and action appropriate.

Please note that all incidents resulting from ordering and/or eating a meal containing an ingredient that you need to avoid due to food allergy, food intolerance or coeliac disease can be reported here.

Our aim is to collate information from FBO’s and the FHS customers and share in a way to create a best practice and prevent another or more serious  incident from occurring.

For the FHS customer  please complete if you, a family member, friend or colleague who has experienced an issue which could have happened or if they have experienced a reaction to the food they have eaten when dining out in any of  the sectors  mentioned above. 

In addition, we are asking FBOs to share their experiences to ensure the safety of all FHS customers. We are NOT naming  and shaming specific FBO’s, but we will share the scenario anonymously to prevent a similar incident occurring.
