Food Business Operations [FBO] Reporting an allergy incident or Near Miss

Issues occurred from 1 January 2025 onwards

Introduction

This DB is to collate incidences relating to Food Hypersensitive [FHS] customer which occurred or could have potentially occurred if they had not been highlighted at the time of serving. The person completing the form can either remain anonymous or add their contact details.  We will not follow up with each person, but if you need guidance this can be requested by emailing office@foodallergyreaction.report

The survey is to collate and monitor issues related to food allergies, coeliac disease and food intolerance to share with hospitality to attain best practice and use as a preventative tool for further risks within food service.  It is not our intention to name and shame business’s but encourage change when issues are identified.  Businesses will remain anonymous if details are supplied, LA can complete the form and anonymize the organisation as appropriate.

In the first instance we would advise you to log any incident or near miss within your Food Safety Management System [FSMS] or accident book, and update the changes made to ensure a there is no repeat occurrence. In addition, you can contact your Local Authority [LA] if further guidance is required.

Anaphylaxis reactions where an adrenalin pen is given, or a hospital visit occurs the venue is obliged to report to their LA and register within their accident book. If the person affected goes to hospital from the venue it should be registered under RIDDOR.

We will not be reporting the issues logged to LA’s, we will however collate and monitor patterns and if we believe there is an imminent risk we will contact the appropriate organisations.

The Food Standards Agency have defined the ‘Food Hypersensitive’ [FHS] customer as someone who consumes or touches a food product which can cause a reaction in the body, including but not limited to an immune, auto immune or digestive reaction.  which includes food allergy, food intolerance and coeliac disease.

What is a ‘Near Miss’? – A Near-Miss describes all incidents that didn’t cause severe harm but had the potential to.  A Potential risk if a business does not identify the Root Cause could cause a severe issue or fatality going forward.

All information supplied will be governed by the GDPR regulations and identities of all completing will be kept confidential.
